Personally I was thinking you should go more of the game design goal of the rest of Rs, create us a place we can explore, and wonder as to what happened there, part inhabited part wild, but everything there for a purpose, or the remnants of a story untold.
For instance the wilderness, or ardogune, or really any other place. You explore and slowly gain a deeper knowing of the land.
Personally I would like a half wilderness half settlement area, maybe some rouge elves or majaharrat followers, a cult of combat dervishes following guthix (think dervish from guildwars nightfall) maybe a ruined citadel or two from the third age, or a town that has ruins from late in the third age, perhaps some caverns and clues to the dragon kin?
That way you could layer in new resources, creature encounters, items, spells, advanced slayer tasks (think the Witcher series of games), more clue scrolls.
There are many places to draw inspiration from outside of the rich runescape lore, Game of thrones, eragon, Lord of the rings, places inhabitted, but once wild, and with ruins whose tales have long since been heard.
Hell even the city of london was at first a lone roman outpost near a riven in a land that was dark and full of terrors.
I think a blend of the danger and suspense and mystery of the wilderness with a pinch of the new and a pinch of the old would be the best idea.
