Yeah, even though I doubt someone will ask in the cc "hey guys how do I leave the clan", where there's a will there's a way I guess and they'd go through the options until they find it.
It makes sense to store the leave button internally, rather than on the clan tab interface itself, because it's not as casual as FC's where you simply enter/leave, and it also definitely solves the issue of confusing it with 'leave channel'.
It does ask for a confirmation to leave clan too.... So I find it hard to believe they left by accident. But I don't want to discuss if they tell truth or not...
Anyway, if it is not too much work to edit the icons or position I don't see why not.

Instead of having leave clan on the interface itself we could possibly put it in a separate window, maybe in the clan details, or clan settings.

There have been members in my clan asking that (low levels of course) I just show the where the door is and wish them good luck (tell them where it's at) XD

I doubted it as well initially, but don't believe someone could make a U-turn in a split-second/instantly and rejoin, especially after it happened to another member again. There's no benefit to them in losing out on all their clan XP, and in most cases people would rather move on if they were thinking about leaving (and had already left) than face the hassle of rejoining.
I couldn't really understand how it's happening but after laying out the interfaces on top of each other (buttons look the same and in the same position), I could see how. The thing is they don't even read the confirmation because when you leave raids groups you leave really quickly as a force of habit and blindly press yes/1 on the confirmation. When you do that a lot, you won't pause after clicking the wrong button (which is identical) and would press yes/1 a split-second after as well due to habit. Especially if it's mid-kill. My deputy who raids a lot admitted that she nearly fell into the same mistake once.
All in all, it's a result of quick (and probably careless) reactions, but with how similar the buttons/positions are, it looks like a faulty design issue which could really do with some improvements/additional checks in place to avoid confusion (i.e. a different icon at least could make a whole world of difference, but more would be safer).

don't you have to still put your pin in to leave a clan ???
Not if you'd already entered it for your session.
No I am pretty sure that leaving a clan you need to enter a pin even if you put in game, I find it unlikely for them to leave by accident!
Hmm I'm unsure about this (would have to try), or whether the latest update for PIN delay affected this. That said, if it's not the case, being forced to re-enter PIN would be one of the solutions, but then again not everyone may have set a PIN.
