A pro tip not many people know. The melee Brothers have incredibly low magic defence, so it is possible to consistently hit on them with magic, even if you are in armour and have a big - magic attack.
How to take advantage of this:
Equip strong melee armour, and a black salamander (magic). Cast ice barrage/blitz/burst on the brother, and step away so you are diagonal to where he is standing.
He will be unable to hit you, but your attacks from the salamander can hit diagonal and consistently with magic, making for safe fast and easy kills.

For me a barrows run is all about spending the least possible amount of money to kill the brothers, here are two tips to save money
1. use bolas to hold the melee brothers in place, they cost 981 gp each, less than the cost of one cast of ice burst and far less than the cost of one cast of ice barrage.
2. you can cut down/eliminate food costs by flashing soul spilt while you have prayer left, if done correctly you lose no prayer points, coupled with ee or eee you should be able to do without food.
I usually end up using 5-10 bolas 0-1 doses of prayer potion and some charges on my cr and polypore staff per trip

tip 1: Always protect/deflect melee on Dharok! When he is low on life points he can hit well over 500 damage!
tip 2: When fighting a melee brother (verac, torag, guthan, and dharok) I sugguest using magic, it is the fastest way to kill them, and some spells can allow you to 'farcast' them making it much safer.

Don't waste inventory space bringing a spade! Since the re-work, they're now permanently wedged in the mounds for your convenience.
Bring a form of Magic & Melee attack - Melee Ahrim and Kharil, and Mage the other (Melee) brothers (Dharok, Verac, Guthan and Torag).

Decide on your speed: Speed is important, so it is important to decide how best to handle a run to get more drops in a shorter period.
1) One thing many do not know is that only killing the brothers themselves improves the chances of a Barrows set piece, hence you do NOT have to kill any of the minions in the tunnels to obtain Barrows set pieces. If obtaining pieces of the sets is your primary goal, it is both faster and easier to only kill the brothers and make a beeline for the chest.
2) Higher Killcount influences the chance of obtaining Blood Runes and Bolt Racks, both of which sell for good prices on the Grand Exchange. Getting Killcount reduces speed, but if you are after profit only and not necessarily the armor pieces, getting a suitable killcount is ideal. A

Drakan's medallion is the most useful item here. Use it to teleport to Burgh de Rott, gear up with melee armor (e.g.: Bandos tassets/chestbody, dragon boots, glory, helm of neitiznot, dragon defender, vine whip, vampyrism aura). Bring at least 1 dose of prayer potion, a restore you get from looting the chest, your medallion, and the rest monkfish. If you can kill Akrisae, bring Korasi (for its special attack) and a few blisterwood stakes (50 is more than enough).
Now that you're geared, teleport to the barrows and start with Dharok. I use Turmoil (or use Piety) on quick prayers since barrows drains your prayer, so you might as well use something until you lose it. If he gets low health and starts hitting high, toggle deflect from melee/protect melee. If Dharok is your tunnel, this is why you brought the prayer restore dose ;)
Now if you can do Akrisae, go to him, starting with you primary melee weapon and switching to a korasi spec when he switches prayers, then back to melee, and finally using the blisterwood stakes and melee to finish him off. Use soul split before specing with korasi if you're low on health.
Now melee the rest of them, eating monkfish when needed and dosing your restore potion if your stats get lowered. Make sure you kill the last boss though! There are 6 original brothers, 7 including Akrisae. If you can't find the last one in the barrows tunnels, try opening (but not looting!) the chest, he might appear. If not, look in the side tunnels that are curved.
Note that killing random monsters in the barrows for a kill count only increases the number of runes and bolt racks you get, NOT your chance of receiving a barrows item! I'm not sure how visages are calculated as they are new.
After you loot the chest, teleport with your medallion back to Burgh de Rott, regear, home teleport to Lumbridge to restore your prayer, teleport to the barrows and repeat! Don't forget to recharge your medallion when it gets low. Good luck!

A common mistake people make is getting cheap kills, if your cheap kills get you slow kills, it's worthwhile to spend a bit more money, as you more than make up for it with the extra chests you loot.
Prayer bonus is somewhat irrelevant with the spirits draining prayer points fairly often, bringing prayer pots however is worth it. As i said before, if you're getting faster kills, you're making more money per hour, so i'd advise pietying/turmoiling and having a high offensive bonus. It's better to protect pray or soulsplit lost hp than to get slow kills.
Fast/efficient method I've used before is polypore staff with dds+rapier for karils and an msb/rune arrows for ahrims.
