Slightly incorrect. The map has never been enlarged. There were lots of black spaces around the current landmass that have since been filled out to the current square, but nothing has gone beyond the current square.
Do a search of the map from say 2007 and you'll see parts that extend out to the current limit with lots of black blocks around them. The only change is that these have been filled in. Map sizes (worldspaces) should correspond to height-maps (a black and white image that contains height gradient information) which uses image pixels sizes to determine the world space dimentions. So the map has always been a constant size it just now been filled out.
Indeed, if you wanted to add a new continent it would be very easy to do so. Because you would never physically walk there on a land bridge and you can't swim, it would be just as you say an easy task of adding a portal - or in the case of new continent probably a ship.
The new elf city does not exist on the runescape map. It seems to have been developed separately and is set in it's own world space. Easier for developers to work with and might lower lag for some players. They've just added a little invisible portal as you pass the elf guards that puts you into the new world space with a little bit of the runescape scenery skirting around the edges. If they increased the draw distance you could probably see the end of the 'skirt'.
Btw I'm just commenting on how the new elf city was implemented I'm by now means advocating that we have invisible portals everywhere to expand the map.. that would be very messy.
