I've found that the person's username is not the determinant that I use to make my character judgment. While it can provide some potential information, it is the things that they say which tend to be the best indicator as to whether or not someone will be a good fit (which correlates with being a long term clan member, which is what I look for).
The names I find most offensive are the ones that impersonate others, for the obvious reasons.
But does anyones username bother me personally? I think very few ever have, though some names can just rub you the wrong way without being outright repulsive.
I have never, and will probably never, regulate the names of my clan members. Well.. unless someone changed their name to deliberately break game rules or be dishonest (impersonate someone else, we've all seen that scam/received those pm's).
