Where else could the spear go? Since her visor covers her face entirely, and there is no mention of her visor opening or being forced open, the spearhead would not be able to go down her throat; the visor covers her mouth. Unless the spearhead was very thin, then it could in theory slip into a breath hole and go down her throat, but I imagined a regular leaf-shaped spearhead. I just don't know what else the spear would do :/

I interpreted this as an attempt to ram the spear down her throat, as in, enter her mouth, which would surely be lethal. I misinterpreted the details, again.

She is wearing mail around the neck, which would stop the spear, but the impact would still be painful.
As for a protrusive breastplate to match the breasts, I’m not necessarily opposed to that. Historical armours did have a protrusive ‘breast’ to serve as additional protection, it's just the rounded section of the front of the breastplate, to cause lances and projectiles to glance off. Also, it’s important to note that armour was tailored to match the wearer’s body. If a woman was to have armour tailored to match her frame, chances are the protrusion of the breastplate would simply be a single rounded section (as can be seen in the picture).
Two round protrusions would be bad, as a lance could snug in-between them and hit the breastplate with greater force and precision. However, there is no reason a woman couldn’t wear armour made for a man; so long as the waist sits on her hips, and the armour matches her hips, she’d be alright. There’s just nothing about a woman that makes a man’s armour unsuitable for her.
As for being struck on the breast, well, it would take some air out the lady, and cause pain, but it wouldn't be lethal.

I'm only wondering if the protrusion would cause the armour to become more malleable (hence why it should be designed like a mans). I imagine leaving dents in form fitting plate armour would ruin someone's day, but then I've never actually tried taking a hammer to one - so I have no idea how tough the stuff actually is.
