Though, if I owned Mass Effect, I'd have Shepard's death something like this: swarmed by many enemies, let's say Reaper-troops, gets armour shot apart. Shepard escapes, but gets swarmed by Husks but also bite and gnaw, and plucking one eyeball out. During fighting, a Brute is killed but rolls upon Shepard, pinning the commander's arm to the ground. Shepard is forced to cut off arm or shoot it off, but tendons hold it together, so is forced to rip it off the old-fashioned way. Continues struggle; still getting swarmed by enemies, being eye-gouged. Surrounded and piled.
Resorts to explosive, blowing up enemies, surviving, but also blasting skin off half the face, loses some teeth, half side of jaw blown off, stomach ruptures and guts hang out. While intestines droop over knees, Shepard pushes them back in, and continues struggle. Gargles blood constantly.
Approaching crash-landed ship; although missing an arm, an eye, half face, and some skull and teeth, continues. Gets shot at; bullets shatter knees, Shepard's legs collapse, knees breaking but bending backwards. Crawls forth towards safety of frigate, Husks rip legs off below the knee. With one arm, and two half-legs, Shepard uses pistol to fend-off remaining foes, crawls towards ship, but fuel leaked out and ignites, putting Shepard on fire. Crawls towards ship anyways, smouldering and bleeding.
Saren, whose body had been upgraded by Harbinger, is recreated with those spare parts; comes back for ultimate revenge. Grabs the bleeding, disfigured, burnt, smouldering, suffering Shepard, throws the commander upon a dragon tooth, and activates it, impaling Shepard. Saren watches with his glowing blue eyes as his old rival tries to pull oneself off the device, while being gradually Husked. Saren grabs Shepard and slowly pushes Shepard farther down the spike. Waits patiently for demise.
Alternatively, slowly melting into a skinless state, and then being impaled, in the pretense of crew mates, would also be good enough.
