Introduce a 'no follow' rule for this minigame. Even if it's randomized players, clan members can potentially get the same game and follow their targets at the same time. They can change combat styles, and if overheads are allowed without the player knowing, you can get combo'd out because of discouraging game circumstances.
I think a small amount of NPC's should spawn along with you at a random chance. A rare chance to spawn demi-boss(s) or just bosses in general that ALWAYS drop something rare. (whip, trident, armadyl crossbow, Godsword maybe?)
Personally, have an array of items that you may find in bushes, shelves, bookcases, chests, NPCS etc.. Instead of just basic items, have the potentiality to find several other items including rare items and or even new items. Add more buildings, more depth. PLEASE make sure it's not as empty and open spaced as Zeah.
Have altars, merchant traders, areas to gather and or craft (natures at least?) for alching to buy from merchants?
Make this about strategy and every game completely random so you're fighting with your circumstances and THAT will make the best last man standing. Put some thought and creativity in your content, it's YOUR game afterall.
