Hi again Cacoadragoon, what I meant by the dueling ring, was that it is usually a waste to keep going until the dueling ring is used up, as you will make fewer runes after 2 - 3 runs to the Nat alter once the pouch degrades. (I understood what you meant).
As far as guides go, there are a whole lot of them out there. The one by Balls0flead is nice, and it is quite useful to watch Phoenixodin's one just to observe how he does it.
A few tips when graahking (not that I am fastest in the world):
1. Have your bank totally full when you are dressed to craft and have your 3 largest pouches in your inventory. That way you just click on the little tab at the bottom of the bank screen to bank you inventory when you open the bank and the pouches stay in your inventory.
2. Use the minimap actively to click ahead of time where you want to go. You can click when you tele from CW just a little bit before you even see the karamja minimap, likewise from the alter.
3. Watch your arm movements when crafting at the alter. As soon as they go down you can do the next action (empty pouches, or tele out). The sound of crafting can also be a good guide for timing.

You know when to repair pouches when you get a message on the text at bottom of screen that your pouches have degraded, and the filling of pouches kinda messes up that time. The pouch also looks dingy or darker.
You get the small pouch in exactly the same way you get the medium and large one. You kill abyssal beasties in the abyss until you get the drop. It can take up to a half hour, so bring food and a tele in case you are in danger of dying. I usually kill the little ones that look like ants as they drop them too, and are easy to kill. - Good luck.
