Chris,
No, no one actually has to keep all those details to track, that is up to the game master. In this case, I would personally be keeping track of everything.

West,
Some details are held later on, as modes can be deactivated. I just reposted the thread, by editing existing posts, so it should be easier to navigate.

OK, I just added a legend, third post, to make navigation easier. There are several reserves throughout the thread, as well as some posts being partly filled only. Examples of fights will be added soon, just need a little break.

Creating this system was mentally straining, so in case I have not been answering queries or questions adequately, feel welcome to ask again or request a more thorough response.

Accuracy modifiers
Eyes -90% to hit
Head -10% to hit
Head +30% to hit (if immobilized, stationary, unsuspecting, or held down)
Neck -20% to hit
Torso +20% to hit
Shoulders +10% to hit
Hands -20% to -40% to hit if opponent wields weapon with protective hilt
Feet -30% chance to hit
All Ranged attacks have an additional -30% chance to hit unless used point-blank.

Sorry if this is explained elsewhere, but how exactly does this work? If I aim for the eye but miss, do I still have a chance at hitting the head? Or do headshots have a chance of hitting the eye? Or do failed rolls aimed at the eye miss the head entirely?

If you aim at a specific body part, there are modifiers to your success. Hands and feet can move quicker than the torso for example, so it's harder to hit, unless the character was stationary or held down.

If you missed the eye, you'd hit the head, yes.

I will add some info about that section soon.
