Okay, I’ll try to explain this the best I can – but I don’t have all the answers, and am open to suggestions or interpretations.
“Sever” is a magic that is entirely conceptual. Namely, it follows the logic of the user, and adjusts its power accordingly. For example, if I picked up a sword and imbued it with the idea of severing an armoured knight in two, I could swing that sword and cut clean through that armoured knight. Likewise, if I only wanted to penetrate the armour, the magic would cut through the armour (though wouldn’t necessarily halt the sword’s motion).
Onto your example. If a name was severed, whether to remove a curse or contract, I imagine that person would begin to lose a sense of self – by hacking at a name, you are essentially ripping out the core and soul of a person. I imagine that person could retain the drive to replace the lost persona, or eventually devolve into a potato. Or maybe nameless, soulless people make better villains? I'm not sure.
Are Alisha’s severs permanent? Yes, but there’s nothing stopping you from negating or recovering what was lost - her cuts are clean, and never truly destroy. For example, if you had a recovery ability, there would be nothing stopping you from simply regrowing another arm or reattaching it. If you hold a severed memory, perhaps going to a place where that memory held weight would jog what’s left of it.
