I picked Dimension of Disaster specially because of what it was - a loosely described setting where everything turned to hell, throwing most established lore down the toilet. Taverly and Burthope are probably the most widely known areas of the game right now, and are (hopefully) the most sheltered from the cataclysm. If we imagine that the mainland were the size of a continent, then the surrounding areas of Burthope/Taverly should give us ample room to play in and explore.
As for mechanics, I guess your character does the best with what they know. It's a whole new ecosystem after all, and I hope there won't be too many Mary Sues running about. Could you cut down a tree with a hatchet? maybe. But ask yourself this - do you have the knowledge or tools to do so safely and efficiently? I could go into another analogy with Herblore/Cooking about mushrooms and wild fauna, but I'm sure that speaks for itself.
If you look at it from this perspective, a level gate is actually your characters common sense kicking into gear.
Rest easy, HP will not be a thing. I imagine, but haven't experienced, that being stabbed in the gut would hurt one guy as much as it would the next. Unless, of course, HP is actually

Will it be a low level thing? That depends on the competence of everyone's characters - though I would like for everyone to remain somewhat realistic. I don't think anyone would be keen to grind on hoards of armoured zombies for the gainz when their own life was on the line. Well, I mean, anyone who didn't belong in some form of asylum. That would be fun to characterise!
Size doesn't matter, it's about the EXPERIENCE! Or the scaling. Small things look big to small people and all that.
