Yes, it's a wonderful idea but there will be many issues that need to be adressed, most of them already being said throughout this thread.

Adding a voice chat in Runescape would be a great idea.
How voice chat could be a nice tool to use?
-As we gain level, we found some friends and we make a team to slain some bosses which is really dangerous for our lives. A single look at the keyboard and you can be killed really fast by any god wars dungeon bosses. Also if you have to tell someone to move or go somewhere in the boss room, the player who have to move will react much more faster than waiting the player to write the text, and after read it and now move.
*Including Voice chat in this context would help to have more fast reaction and less deaths in your dear joyful game.
-At dungeoneering, it's a real useful tool. The keyer don't have to spend time to write anything on the keyboard so he can handle his two hands where it's suppose to be! On the mouse and on the arrows/F-Keys (F1-F3-F5). It would help the keyer to claim keys which he don't have and ask for help even faster.
*The dungeon "time taking" would be reduced in any ways and it will put good moods inside every players in the world.
***Technical Stuff***
Voice chat should be only used in friend chat and clan chat, but here is the most unexplained problem...
"How could we use 2 voice chats at the same times without confuse anybody AND without have any echoes in our microphone?"
Well, my solution would be to configure WHO would you like to hear in the friend chat and clan chat. Like a friend list but it's for voice chat, and put both voice chat in one.
OR
Only use one voice chat at the time. Even with that, it would be a trouble.
Best solution: Make a new "voice clan" option. With that, you can invite your friends in a voice chat session which a owner as been choice and it would be like a "3rd clan" only for voice chat.
Note: You can name the "voice clan" as any ways you wish. It's only an example.
My conclusion: The voice chat would be an excellent way to boost the gameplay quality of Runescape.
