If you're on RS3, you can find the entrance to Death's Office near the Draynor Lodestone. However, if you're playing OSRS, you can find the entrance to his office near the following respawn points:

Lumbridge graveyard
White Knights' Castle Crypt in Falador
Seers' Village graveyard
Edgeville ruins
Hefin district in Prifddinas

Its location is indicated by a tombstone displayed on your minimap.
