Its not difficult, rather simple, the main thing to do is Don't Die . Dieing tends to slow down the boss hunts...

I think I've done the quest but I've never fought the boss
So is the quest battle differnt than the boss battle?

You don't fight Nex during any quests, so if you wish to fight her you have to enter the ancient prison in GWD, which requires 70 strength, constitution, ranged, and agility since you need the frozen key pieces. As for the boss fight itself, you pretty much need the best or close to the best gear available.
