I think
The Fremennik Isles
should be a Fifth Age quest, because it is required for Glorious Memories, that is a requirement for Blood Runs Deep, which happens before Koschei's Troubles...

Koschei's Troubles isn't neccessarily Fifth Age, could easily be very very early Sixth Age.
You never were our brightest star, Khazard. 'Vermin slaughtered like lambs'? What does that even mean?
True, but still, nothing outright says it has to be in the fifth age.

To this day, I still wait for the subdivision of quests (like those for Progress, Free and Members, and Difficulty) in 5th Age or 6th Age related quests, it'd be so easier for new players to pick up the lore elaboration pace. Also, a subdivision based on related civilisations and different story lines of RS.
I'm going to have to disagree with some of the "either age" ones. A lot of them mention gods as have been being away, even if it was in a throw away sentence. The Blood Pact comes to mind.
EDIT: A couple other reasons.
I feel Nomad's requiem more than likely takes place in the 5th age. Nomad references god powers as if they're rare, and it's also implied that he's in alliance with Lucien (also shown with him getting confronted by the elite guards in the beginning), which would obviously mean this took place prior to or around ROTM.
Meeting History could be debated, seeing as you learn about the past; But it'd seem that The World Guardian would know about Guthix, instead of being dumbfounded by him.
ANOTHER EDIT:
Do No Evil talks a lot about the desert pantheon. I'm sure there's something in there that makes it 5th.
