Have been in a few over my 10+ years of playing. Most recent clan I left because of drama with an IRL friend & member that invited me. All others have been from drifting away from the game.

Yes, I didn't really connect with them. Some of the requirements and rules felt way too crazy for a clan that's supposed to guide midgame players reach the end game when the majority of content done by the clan is bossing. I came back to the game from a long hiatus and felt little to no support from veterans.

Yes a couple years ago - I had to leave due to taking a long hiatus from the game because of grad school demands
