How to increase difficulty gradually? Alright. Two quick ones.
First of all, do away with hubs and lodestones entirely. Make more respawn area rewards or default respawn choices (quests, achievement diaries, etc) instead and allow magic teleports to do their job (this will also make law runes more useful).
Secondly, eliminate gravestones. Keep the same item-retention mechanics (3 items of your choosing + minor items). Or, if you feel gravestones must be kept, the duration on all gravestones should decrease with your combat level. This would allow new players time to familiarize themselves with RuneScape combat, but the more you've experienced RuneScape combat the less excuse you have to run in to an unknown situation (or even a known situation) without caution. I know that losing gear can be painful (it's happened to me), but it also allows for items to exit the game so they can retain their value better. 15 mins is overkill to get back to your gravestone (and blessing graves makes it virtually impossible to not get back on time). There should be a sense of urgency to get back to your grave. Player death should be more than a minor inconvenience.
