I used uhh...

Full crystal armour, noxious scythe, then the usual BIS gear. 1 holy overload, 5 prayer flasks, 1 replenishment potion, excalibur, spring cleaner and the rest was rocktails - with a blood nihil.

Nobody knows the drop rate, but wearing crystal armour (each piece raises the chance of you receiving it), with attuned helping twice as much per piece - but I opted to kill more and use the crystals to sell the then expensive crystal armour, rather than buying attuned armour with it.

I'd imagine ~500 kills if you were to buy Attuned armour as you can afford it, and around 800 without. Unslain Bosses - Clan

Sacrifice method confirmed working for the most part.

I died at Araxxor and did not appear to get improved loot. I guess it's up to the higher power above whether or not your death is worthy enough to trigger better loot for future kills .
