I've always thought it would be a good update to have rockfalls in regular dungeons, and rockfaces to climb within the cave.
As an update to mining and agility. Clear the rockfalls with mining lvl, and scale the rockfaces with agil lvl.
The higher level you are in both these skills, the deeper you can go in the dungeon, and the deeper you go, the higher the rewards.
Theses caves could have a multitude of resources, NPC's etc, and only the best in mining and agil would be able to access best rewards.
Potentially you could add other skills into the equation :
1. Dungeoneering resource dungeons deeper into the dungeon, or even dungeoneering level required to go deeper.
2. The use of construction to build bridges over chasms.
3. Locations for better concentrate ores.
4. Locations for new fishing locations.

Maybe add in a room with furnance and anvils, where you would need high smithing to make a key to the following part of the dungeon, but that's a bit too unnatural compared to other things, and maybe too dungeoneeringish.
