It's an anomaly. In practice, irl, my friends and I can pile anyone without messing each other up in fencing, airsoft, etc. The moment all the fighting becomes text, blunders appear that don't exist otherwise.
And yes, them not specifying when one move happens in relation to the other, is hectic fun if the other side isn't an ass about it - lack of detail is always the fault of the person making the action. A friend of mine, back in '11, was fighting on the Ardougne bridge, and had a whole row of people attacking him from it. He quickly figured out that most of their attacks would likely hit their own friends, and due to poor group coordination, stopped almost the entirety of their attacks by stepping back and raising his shield. OOC confusion, people walking in each other's way, and other blunders did the rest.
I haven't seen that as much either, but it's a rewarding mistake on their end if you can explain to them how everyone can still move, and why in most cases they just walked into an attack. (Of course, them acknowledging that they majorly messed up, has happened... a very low amount of times.)
