had all 12 and 4 in my inventory when I charged the device
only seren west door opened so I can't get the zaros cosmetic
I've tried to re-gather the crystals but they haven't reset/still register as being removed/taken.
any idea on how to fix?

Check the device. Make sure it's registering that you found all 12 zarosian crystals.
