Time is a funny thing. The theory of general relativity states that time is relative to one's location. However differently time is flowing, you won't realise it until you go back to your original timestream.
He ages at the same rate wherever He is, so His age is not set in Earth or Gielinor years. Even when He is in Gielinor, where time is moving extremely fast, thousands of years seem like thousands of years. In addition, the human body does not recognise time differences except at the moment of change. Thus, the human body ages no differently, no matter what the time stream.
Guthix is millennia old, and, had He not chosen to remain the same age in appearence, He would look it. Guthix choses to appear His age if He had never been in Gielinor, currently 15.
