Randomly dissappearing attacks

So I'll have my attacks the way I want them on action bar 1, and it'll just randomly remove them at random times. I am doing dungeoneering and for instance, I had double swing and kick assinged to 2 of the buttons on the left side. I completed the dungeon and those 2 were gone in the next one, so I had to reset them. Shortly after, it randomly removed kick again only I think that was actually mid dungeon that time. Anyone else having this glitch? It's so annoying I don't think it's worth playing for me until it gets fixed... "For all you know, I'm the silver in the bullet
