There has been an issue where these forms don't "submit" but I haven't heard of anyone having a problem recently.

Try clearing your browser cache: How to clear your browser Cache
If that doesn't work, try submitting from a different computer.
Lastly, wait and try again tomorrow.

The payment date should be exactly the same each month. Are you reading the 'Date Processed' or the 'Date of Transaction'?

It would help if they put DD/MM/YYYY or DD/MM/YY or MM/DD/YYYY or MM/DD/YY or something else
If they say what they want I can give them the correct answer, as they do not want the right answer.

It's a bit like those questions you read 5 times and the right answer is Yes and also No. If you simply say what you actually want the answer will be Yes or No not both or in this case the payment date.
