Jagex won't resend any emails to give you the information you lack.
The payment section of the form is only for purchases made directly from the website with real world money.
If the Bonds you redeemed for membership were purchased inside the game with gp, that information can be put into 'Other Comments' but won't go into the payment section of the recovery form.
Don't submit your Bank PIN in the form - it cannot be checked.
Why have you lost access to the account if you still have the password?
Is Authenticator keeping you out?
Are you checking the reasons for the deny of your recovery?
How quickly does the deny email arrive after you submit your form?
