I have a theoretical scenario to put to you and I want to see how you think it would play out.
We all know that Harold was chosen as Death as the first human to die on Gielinor and theory would have it that it was Guthix that did the choosing. Though other sources would say it's the first sentient to die that becomes Death, to shepherd others, a natural occurrence. Ingore the Guthix choosing for now as a myth.
Here is the scenario:
The World Guardian goes back in time, no one else has arrived yet. Guthix may very well be stomping about, but that is it. We meet an untimely demise on arrival. So we are the first to die, not Harold.
What happens next?
The purpose of adventure is to shine light into dark places,
Poke monsters with a sharp stick, Then steal anything that isn't nailed down!

Yet that would mean there is a fate choosing who's time it is. Ol Harold could have been chosen after and giving a list of limits as the collector of souls ready to pass on, rather than being a Reaper who choses who's time it is.
That still leaves us with the question of what happens next.

The way I see it is that Harold became Death when he died of old age (as the stained glass depicts). However, Guthix saw that this was a lot of power for one person, so he bound his power to the scythe. I don't think Death's restrictions are natural, rather that they were placed on him by Guthix to maintain balance. Look at his dialogue in The World Wakes
When Guthix appointed me as a guardian, I was given the task of retaining the balance of life and death - as well as the power to do so. With all great power, there must be balance, so Guthix
also bestowed a weakness upon me
.
Of course, this was probably before the newer Death lore, notice how he says Guthix gave him his powers, but it makes sense. Perhaps Death merely believes his power comes from Guthix.

How do you know they're not ready to die when we kill them?

There could be an even more bizarre explanation. Something which dies and has no chance of passing on. Something we would be uniquely qualified to do. We can channel a certain energy without being consumed by it. Being mortal ourselves, even in our odd way might be critically important. In training as the Death of gods.....I know a little out there, but hey, it's a theory
