Similar to what you did with Fresh Start, I'd like to propose creating worlds with rules similar to the last leagues. That was immensely fun and for someone like me who's skill set isn't remotely considered sweaty, it gave me a chance to do some things I haven't been able to do in the regular game.
To make this a permanent type of world, I'd continue with points needed to unlock all of the regions and possibly even allowing dual relics from some levels (or being able to change with a point cost). Group ironman would be awesome, but not completely necessary. Points would NOT be able to be used to buy the league rewards. Just unlocking gameplay within the league world itself.
These worlds would require brand new characters so it's a potential new revenue stream for Jagex.
Bottom line, you did an INCREDIBLE job with the last leagues. Thank you very much for your effort in putting them together!
