How about a outfit from the front you look like one gender and from the back you look like the oppisite gender. So the front my be female while the back is male or the other way round. When in game and people look at you they won't be able to tell if they are actually looking at your front or back. You will be able to switch the outfit around, so you can choose if you want your front to be female or male.

There could be an emote wth him/her saying a pun while dancing or walking both ways, so the spectators (other players) won't know which way is which.

I'm sure the graphics team could create a character to represent this idea. The character does not need to be 'normal looking' it can be strange, weird, abstract - something different, unique.
