I did the combat academy but had a disconnect during the survey at the end, does this mean i wont get the reward after actually doing what actually was listed?

I wasnt able to get to do survey again after the dc.

unfortunately, I did not receive the mad scientist outfit, even though I have participated in 5 of the beta events
how can I fix this?

I definitely did the first four events (not the makeup ones that were scheduled though), no outfit for me.

I did get my XP. I guess I will wait a bit longer as it could just be a slow rollout

Mod Mike : Please bear with us as we're still in the process of rolling out both rewards, it's likely that you'll receive the Combat Academy XP first and the Mad Scientist Outfit later. You should be receiving both, assuming you're eligible, within the next few hours.

The Thanksgiving event this year was ran by the community team rather than being a content developed event. However, that's not to say there won't be future opportunities to unlock the Give Thanks emote.
