To be more precie, the label means we have "drug" and "in game purchase" in RS3 and OSRS.

Drug refers to the consumables like potions that can enhance the abilities/stats of our characters. We don't have the real world drugs in RS3 and OSRS.

We do have in game purchases, or commonly known as MTX, in both RS3 and OSRS that we can buy in game stuff with real money.

It's referring to all the booze (real world alcohol) rather than pretend potions I'd say. The potions may or may not be hinting at certain substances that exist but the alcohol in Gielinor is the same as on Earth.

If you go to https://play.runescape.com/ then hover your cursor over the drugs warning, this text will display:

" The game refers to or depicts the use of illegal drugs, alcohol or tobacco".
