It could simply be every 10k (or so) damage he takes, he goes under. So if you normally need 10 hits to get there (or whatever) you don't notice that hard coded threshold, but when you do it in one hit, you notice he instantly goes down.

Har-Aken's head never appears on the north side of the platform. It randomly surfaces at the east, south, or western sides of the arena for 20 seconds before going back under for another 50 seconds.

Also, I have done +10k damage with onslaught which didn't force har aken to respawn. Either it's a bug with the bolts you mentioned or you are in the wrong
