A holy symbol a friend of mine gave me, somewhere around 2005.
I originally had it on my first account, where I met her, but that account was hacked. Years later I managed to recover the account and transferred whatever I could to this one, and the first thing I transferred was the holy symbol.
I guess it's just very special because of who gave it to me. The last time my friend logged out was in early 2006 if I remember correctly, and I haven't heard from her since. I was 13 at the time, and she was the reason I'd run to my computer after school hoping she would be online, but that never happened.
I really, really miss her. I hope she's okay.

My favourite item would be PartyHat. Not only because these are one of most expensive items, but I remember back in the day when Blue one was 160M-ish and that was considered rich back then. You were able to buy PartyHats with like 99RC goal or something similar - was achievable, but hard to get still.
Now they're expensive as hell.
I still do not own one though - always far away from being able to afford one.
The favourite I actually own, should be my Hunter SkillCape. Got it years ag at Red Chinchompas and I have some great memories of that era of RS. And Hunter (especially Box-trapping) relaxes me.
I still wear it often although I've achieved many other Capes of Accomplishemnts since then
