What I am suggesting is that for each level of achievements, for everything within that level for all areas of that level, there are achievements that reward you for using the achievements a set amount of times, and the rewards for doing these, areas compliment other areas, perhaps the rewards for doing these is greater amounts given of the things you collect, more teleport locations, more shortcuts, better chances of things they do sometimes happening, maybe activities you can't access without them, perhaps places you can't access without them, things you can only do if you have all achievement of achievements for a level of difficulty.
For the place I suggest the Achievement Guild which has a different floor accessible for each difficulty, and perhaps it acts as a hub of a sort with all the teleports to the collecting locations, maybe even some of the npcs you collect from.
