Short version: Bug(?) in slayer souls.

Long version: I keep dumping abyssal demon souls into the box at slayer pyramid, everything's good for a bit and then the kill counter informs me that I've not collected an abyssal demon soul yet/codex goes back to uncollected status. Very frustrating.

The kill counter issue could be because you don't yet have the souls from all the new types of abyssal slayer creature - e.g. abyssal savages, abyssal beasts and abyssal lords.

But if it's reverting your codex for abyssal demons, then it's clearly a bug, so report in game.
