Reduced combat effectiveness

I have a problem with my combat attack apparently reducing. I've managed to get to level 138 (1960 skills) but haven't really bothered to learn much about combat settings and stuff. However, recently I've noticed it can take much longer for me to kill average monsters. At the moment I'm on slayer task for desert stykeworms. I used to dispatch them in 30 seconds or less, but now it's taking me over a minute for each one. I've tried everything I can with various settings, but nothing seems to work. Has anyone got any ideas? Am I out of the loop with something obvious?

If you are using augmented equipment, make sure you have charges in your charge pack.
