Death should always keep your items for a fee or any item recovered for a fee and they will never leave death's domain unless recovered, and will never disappear from death's domain when player versus monster.
The problem here is all the backend storage requirements that would be needed to treat death like a secondary bank. This is why the payment mechanic exists so you can buy back your items if you don't wish to go loot your own grave to get your items back.
So if you can easily reach your grave if you perish then visit your grave to retrieve your items, there's no fee there. However if you die again prior to looting your grave, the original items are lost to the void. This is why the fee at death exists to begin with to mitigate the losses of items.
If you think it's bad now imagine back to the era where when you died your stuff fell to the ground everywhere for people to loot. After like 30 seconds and gone to the void in under like 2 minutes.

ahh the good ol days got a free set of bandos cause of this guy died at hell hounds in kuradels dungeon after finishing hell hound task cause he was afk and he couldn't get back in without being on a task in the dungeon that of course was with gravestones added but still
That's another reason y we claim our stuff from death so something like that doesn't happen again
