In my summary section I have the following listed in my "Quest Progression".
Completed: 289/295
Inprogress: 2/295
Not started: 4/295

When you go to the Quest tab, it states I have completed all quests for 413/413 quest points.

Question 1: Why does the summary section not match the Quest tab?

Additional info: I do have one quest labeled yellow but have completed it. "Dimension of Disaster". I think that is yellow because I was going through some of it again to complete achievements for that area.

I was thinking maybe mini quests or something but cant find anything I have missed. Almost done with Master quest cape. Just kill beast left.

Thoughts on issue?

This would be the problem.

Replays of DoD effectively 'reset' the summary section both in-game and on your Runemetrics profile, although the total amount of quest points will remain unchanged. Play through the Quests as normal, and when they're all done, your total amount of quests should be back to normal.

As you need to play through DoD at least five times to unlock all the rewards & achievements, it isn't uncommon to see this, and you're likely to see this occur in the future if you reset it again.
