You have to claim the reward from Twitch and then from RS:
Find the offer on the Twitch Prime Loot page.

Click on the link in Step 1 to link your Twitch and Runescape accounts, even if they are already linked.

On the Runescape webpage (you may need to log in) , click 'Confirm'.

I have messed with it over and over and it just will not work. I redeemed something from Twitch Prime months ago and it worked fine. It keeps telling me Oops!
In order to claim your loot you need to sign up for Twitch Prime and click claim on our offer. Well I have done that 20 plus times and it just continuously loops me. Then I tried unlinking my account and relinking and now it won't even let me relink it thinking the darn thing is "Connected to another Runescape account." No I have only one runescape account. This exact same problem is happening to my boyfriend's account as well. What the heck?

If it helps, the Twitch Prime FAQ - Linking Accounts (click me) support page does contain some useful information about linking your accounts and claiming the loot. As Tren described, there's a couple more stages that you'll need to follow after clicking claim.

However, the linking of your accounts may be the main issue here. From what you described, it sounds like you unlinked RS from your Twitch account. Instead, you should also try unlinking your RS Account from Twitch in Account Settings (as described in the article). When that's done, relink it. You should also make sure you have the correct Twitch account linked: I've seen this cause errors such as this in the past.
