Trailblazer Harder Modes?

Was just wondering if anyone can give some clarification on how to access the "harder" modes in trailblazer (for raids, phosani's nightmare, etc.). Particularly, I was trying to figure out how to do Nightmare's Nightmare; I've done the regular Nightmare as well as a few runs on Phosani's and don't see where there would be an alternative option to do the harder version. If anyone has information on that as well as the higher difficulty raids, I'd appreciate that too.
