There's several factors at play here, the biggest being the priority of how detrimental the "broken" feature is to the service.
- Avatars were very nearly almost completely broken at one time (not too long ago, in fact) and were fixed as an unintended result of something else being fixed.
- Titles have been unavailable on the forums/website since at least on or around the October 2018 titles issue when they were disabled in-game also due to bug that caused player profile corruption. While this bug has been fixed in-game and titles were re-enabled in-game, they still do not display on the forums or website where they once did.
- The forums, website, and game all continue to work normally with or without the avatar and title features, so the priority assigned to these issues is likely very low.
- The in-game title feature was prioritized higher due to players socializing in-game with other players and being able to "show off" the title. While the forums do retain some activity, the comparable volume of traffic and use with other platforms does seem to indicate a preference towards those platforms instead which leaves minor, non-detrimental QoL fixes centered around the Forums in a lower priority status.
- To fully "fix" the avatar issues, I would guess that we're probably looking at something to do with the game engine and how it generates an avatar graphic from the in-game models of the player and wearables. Prioritizing that fix is going to depend on what Jagex has in vision for the future of their website, forums, Runemetrics, and other services that utilize the avatar graphic in conjunction with how important it is to the usability and/or reliability of those services, and while keeping in-mind the user desire to have this functionality continue.
