Following today's update, it appears death costs are now broken. Retained items are now based off of the order they are appearing in your inventory, and not the intended way. Tested this a number of times also.

Runescape Twitter says Free Deaths are activated.

HOTFIX: We have enabled Free Deaths while we investigate an issue with Death Costs.
